A common question we get asked at Clemente Orthodontics by our adult patients is, “Is it possible for me to get a wider smile?”

Unfortunately, without surgical intervention, it is very difficult to widen the arch of an adult patient without causing bone thinning or recession. This is because the adult skeleton is fully formed, and true arch widening requires the bones to be actively growing.

What is Slow Growth Modification?

Although we cannot widen an adult maxillary arch, we can widen the arch of a growing child, preferably between the ages of 7 and 11.

This arch widening is often done during Phase 1 orthodontic treatment, which comes before Phase 2, the actual tooth straightening part of the treatment. Here at Clemente Orthodontics, we do not believe in the philosophy of rapid palatal expansion (RPE). Instead, we focus on slow growth modification.

As the child’s bones gradually grow, we can gently modify that growth with them. Slow growth modification is less intense and more stable than RPE. 

How Do We Do It?

Our signature appliance is called a GMA. Dr. Michael Clemente actually wrote the protocol on how to use this appliance, and we have been successfully using it for over 30 years.

The GMA is a removable retainer that is split in two directions with small screws. It is expandable and comes in 50 different colors and patterns. This appliance is designed to work side to side, like a traditional RPE, but it also works front to back and vertically. It works in three planes of space, something that a traditional RPE does not do.

This appliance is only adjusted once a month by our team at Clemente Orthodontics, and each appointment only takes about five minutes. The adjustments translate to about a 1/4 millimeter, which means they are hardly noticeable. Our younger patients tolerate this very gentle appliance extremely well and appreciate the quick appointment times.

The GMA creates a wider arch and room for the permanent teeth to erupt properly. In some cases, it contributes to a fuller-looking smile by revealing more of the lower teeth.

Does Everyone Need a GMA?
